Informed Consent Form – Health Practices

This Informed Consent Form for Health Practices is curated for the business owner who is conducting health services, health practices or medical treatments of any kind and is seeking informed consent from their client or patient. For purposes of clarity we referred to your client or patient as a Participant throughout the agreement (feel free to change it!).

This legal document is what you would have your Participant(s) sign prior to their participation in the services to give informed consent and acknowledge any risks.

The Informed Consent form includes:

• Participant’s Declaration
• Practitioner’s Declaration

By signing this Form the Participant understands that:

• They are responsible for any potential risk(s) associated with their personal participation in the services(s)
• They are responsible for their own health and well being during the services(s)
• They are responsible for any physical harm that they could incur during the services(s) (no matter how unlikely, we have you covered!)
• They have been informed of what the services entail and will promptly inform you if they feel any discomfort/pain during the services(s)
